Customize your form

Change the title, instructions and fields, then download a reusable HTML file. Edits stay in this tab until you download; no answers are collected by SimilarForm. The downloaded file lets you fill answers, save an editable copy and print to PDF.

Replace bracketed details before sharing. Use a monitored contact address and realistic response times.

Edit field labels, choices and required marks

Required marks are guidance on the local file; it saves drafts without blocking incomplete answers. This file has no submit destination or automated booking workflow.

Prepare with the same evidence

Enter the employee, role, reviewer and actual start and end dates. Ask the employee and reviewer to bring the goals agreed at the beginning of that period, examples of completed work and any changes in scope. Describe the work rather than a personality trait.

For each goal, record the target, observed result and evidence. Keep a missed deadline, a changed priority and a missing approval separate so the next action addresses the actual problem.

Make the assessment understandable

The five choices distinguish exceeding, meeting, partly meeting and missing agreed goals, plus insufficient evidence. Select the closest description and explain it. You can edit or remove this field if your organization uses a different approach; agree on definitions before assessing people.

Give the employee space to correct facts or disagree. The discussion acknowledgment records that notes were reviewed; it is not a signature or an assertion that everyone agrees. Use your organization’s process for any formal employment decision.

End with a plan someone can act on

Write each next-period goal as an outcome with an owner and due date. Include manager actions such as arranging training or removing a dependency. Put the next check-in date in the record and send the agreed notes through your normal private work channel.

For a midyear or annual review, change the title and date range and expand the evidence window. You do not need a new template every quarter. Start from the blank download so a prior employee’s answers do not carry into another review.

Completed fictional review: Q3 2026

A customer support specialist met two of three goals. One article remained blocked by an approval dependency. This example was created October 15, 2026; all people, dates of activity and results are illustrative.

Goals agreed at the start of the period1. Keep median first reply under 4 business hours by September 30. 2. Publish six approved help articles by September 30. 3. Complete escalation training by August 15.
Results and evidence for each goal1. Median first reply was 3.6 business hours across 410 assigned tickets in the fictional Q3 queue report. 2. Five articles were published; the sixth awaited product review after a September 18 change in scope. 3. Training finished August 12 and Jordan handled three observed practice escalations.
Overall progress against agreed goalsPartly met agreed goals
Reason for the assessmentTwo goals were met. The article goal was one short; the review dependency contributed. We will separate drafting and approval dates next period so Jordan’s work and the approval handoff are visible.
Strengths and contributionsThe August 22 escalation summary gave the next shift a clear account of steps already tried. Jordan also shared two reusable reply drafts with the team.
Improvement area and support neededFlag article approval delays earlier. Sam will provide a named reviewer and a weekly review slot rather than leaving approvals in the shared queue.
Employee perspectiveI agree that I should flag blocked drafts sooner. The final article was ready on September 16, before the product change. I would like to practise more complex escalations.
Next goals, owners and due datesJordan: finish the remaining article draft by October 16; Sam: arrange approval by October 23. Jordan: publish four further approved articles by December 18, with Sam reviewing drafts within five working days. Jordan: keep the same reply-time target through December 31, reviewed against workload at each check-in.
Development action and manager supportJordan will shadow two complex escalations by November 13. Sam will schedule a mentor and protect two hours for each session. Discuss one learning example at the next check-in.
Next check-in date2026-11-03
Discussion acknowledgmentJordan and Sam reviewed these notes on October 6, 2026. Jordan’s clarification about the article date is retained above. This records discussion, not a legal signature or agreement with every assessment.

The assessment explains the unfinished goal without hiding the manager’s dependency. The next plan gives that manager an action and deadline too.

Questions & answers

Can I edit this quarterly review form without an account?

Yes. Use the customizer below, download the HTML form, and open it in a browser. Edit labels and instructions, type answers, and choose Save editable copy. Print / save PDF creates a printable copy through your browser.

Is the completed review a real employee record?

No. Jordan Lee, Sam Rivera and Harbor Desk are fictional. The example covers July–September 2026 and demonstrates how to connect evidence, an assessment and a next-period plan. Its numbers are not benchmarks.

Can I use it as an annual employee evaluation form?

Yes. Edit the title and period, review your organization’s definitions, and replace the example evidence. A different review frequency does not require a new article or a year-specific form.
